An exploration of Advance Care Planning: Lessons from the Churchill Fellowship and beyond

Advance Care planning allows us to reflect on what living well means for us right up until our final day. It can also allow us to plan for the ‘what ifs…in life’ as well as our own death. What part can social workers play in this burgeoning field of advance care planning? Where the emphasis is not on medical treatments per se but a conversation about living well and dying well and making sure our choices and wishes are known and respected.

Bio: Joy Cocker

Joy Cocker has a Post Graduate Degree in Social Work from the Australian Catholic University and worked as a clinical social worker in Canberra Hospital before joining the Advance Care Planning Program (ACP) in ACT Health in 2011. In 2014 Joy was awarded the Dr Dorothea Sandars and Irene Lee Churchill Fellowship to explore ACP Programs which respect and honour people’s wishes for health care including at the end of life – Canada and USA. Joy has presented widely on ACP including in 2015 at the AASW National Symposium in Sydney. In 2016 she was appointed as an ACP specialist to work on a research translation project with the Cognitive Decline Partnership Centre in Sydney on improving ACP for people with dementia. Joy has a special interest in investigating how the experience of death of loved ones can be a positive experience in helping people live an enriched life.
